Student Vaccination Clinic

The Akron Children’s vaccine team will be in our district March 26th to administer immunizations to those students only with parental/guardian completed consents. Participation is voluntary. All students 11 years old and older are eligible to receive the following vaccines; TDAP, Meningitis B (only for 16 years old and older), Meningococcal ACWY (MCV4), and Gardasil (HPV). Please keep in mind per Ohio Department of Health it is required that all 7th graders have TDAP and Meningococcal (MCV4) upon entering 7th grade and all 12th graders must have second dose of Meningococcal (MCV4) to enter 12th grade.

Vaccines are covered by most insurances, and at no cost for those who are uninsured. Please check with your insurance provider to confirm vaccine administration is a covered service at school. If you would like your student to participate, please print and fill out attached forms and get them back to me by March 20th, 2025. No late forms will be accepted.
