Quick Med urgent Care is working with four local community school systems. They are applying for grant monies to improve wellness and health within all four communities. Crestview High School Community Service Forms

Crestview High School Community Service 

Students at Crestview High School are encouraged to complete 25 hours of community service hours each year, for a total of at least 100 at the time of graduation. By doing so, students can earn 1/4 of a credit each year totaling 1 credit community service on their transcripts upon graduation.

We have put the form online for your convenience. The form should be filled out entirely with no fields left blank.
