Dear Parents/Guardians:

The Crestview Local School District has been working closely with the Columbiana County Health Department and will act as a POD (Point of Dispensing) site for the H1N1 vaccination of students and possibly their younger non-school aged siblings (ages 6 months to 5 years) who attend Crestview. Your child is not required to receive the vaccine, it is entirely voluntary. The vaccine is free. Dates and times for the clinic will be made available as soon as a date has been determined. Please watch for notes home, the local news and newspaper as well as a phone call from The Education Connection for more information. You may also access the Crestview Local School District website for updated information at www.crestviewlocal.k12.oh.us.

The Ohio Department of Health is encouraging people interested in receiving the vaccine to register online at www.odh.ohio.gov. Registration will also be available at Crestview on the day of the clinic. We will post the registration form on our website, and copies will be available in each building office.
