PARENTAL USE AGREEMENT
FOR
RS-1000 ON-BOARD CAR MONITOR

The following are the terms and conditions under which you agree to accept and utilize the RS-1000 Car Monitor:

Crestview School District Responsibilities

     1.     The Crestview School District will provide the parent of any student driver who possesses a driving permit issued by the District and maintains driving privileges in good standing one RS-1000 Package free of charge for their use during the time that the student is enrolled in the Crestview School District.

     2.     The Crestview School District will not deny a student a parking permit to park on school grounds based on the parent’s decision not to accept the RS-1000 Package.

     3.     The Crestview School District will receive the data collection sheets provided by the parents on a monthly basis and provide them directly to the Governmental Safety Task Force.

     4.     The Crestview School District will not review or use data collected from the RS-1000 Monitor in any way.

Parent Responsibilities



     1.     The parent will be responsible for installing the RS-100 On-Board Computer in accordance with the installation guidelines provided by Road Safety International.

     2.     The parent will be responsible for installing the accompanying Base Station Software on their personal computer and will monitor the information recorded by the RS-1000 On-Board Computer and provide the Crestview School District with information on the data collection sheets provided by the school on a monthly basis.

     3.     The parent will return the RS-1000 Package in good working condition when the student driver either graduates from high school, withdraws from the district or voluntarily or involuntarily relinquishes his/her parking permit. Failure to return the RS-1000 will result in an invoice issued to you for the cost of the monitor.

     4.     The parent will be responsible for all damages and/or repairs to the RS-1000 On-Board Computer monitor that is incurred while the monitor is in the parent’s possession.

Your use of the RS-1000 (“The Product”) is entirely at your own risk. By accepting this product, you assume all risks associated with its use, including any and all risk to your automobile and/or personal or home computer being damaged or malfunctioning as a result of the use of this product. To the fullest extent possible under applicable law, the Crestview Local School District expressly disclaims all warranties, including without limitation warranties of merchantability and fitness for a particular purpose with respect to the product. The Crestview Local School District Board of Education makes no warranties or representations of any kind regarding the accuracy or completeness of any information provided to you by Roadway Safety International or made available to you on the company’s website and makes no representation regarding whether the product or any information related to the product is reliable, adequate, complete, up-to-date or applicable to any particular situation or that the product will perform in the manner in which it is supposed to. The Crestview Local School District shall have no liability for any direct, indirect, consequential, special, or other damages suffered by any party (regardless of whether or not such other parties have been advised of the possibility of such damages) resulting from use of the inability to use the product. The Crestview Local School District does not warrant or represent that use of this product will prevent accidents or maintain the safety of drivers using the product. In no event will the Crestview Local School District be liable for any problems associated with computer equipment or software, or for any circumstances beyond their control including, but not limited to software and/or equipment failure, computer virus, or failure or interruption of electrical or other utility services.


I further understand that in conjunction with the State of Ohio Governor’s Highway Safety Task Force, the Crestview Local School district has agreed to offer the RS-1000 Car Monitor in an effort to promote the safety of student drivers and that its participation in this program does not necessarily constitute or imply its endorsement or recommendation.


I have read and understand the terms and conditions of this Agreement and agree to abide by my responsibilities as outlined herein.
